PAANO ANG MAG-APPLY SA SCHOLARSHIP?

Step 1
Maghanda ng 2×2 Photo at kumuha at fill upan ang Individual Information Record o Application Form sa Office of the Mayor o magparehistro online sa https:/ /bit.ly/LSPBatch6.

Step 2
Kumuha ng Barangay Certificate of Residency at Barangay Indigency Certificate sa Barangay. Kung ang mga magulang o tagapag-alaga ng aplikante ay isang Goverment Employee, Income Tax Return ang kailangang kunin.

Step 3
Dalhin ang Application Form, Barangay Certificate of Residency, Barangay Indigency Certificate / Income Tax Return sa Municipal Social Welfare and Development Office (MSWDO) at magpa – interview para sa Social Case Study Report, at kumuha ng kopya nito.

Step 4
Isumite ang lahat ng kailangang dokumento sa Office of the Mayor

MGA PANGKALAHATANG KWALIPIKASYON

Kailangan ay mamamayan ng Lingayen ang aplikante.
Ang kabuuang taunang kita ng mga magulang o tagapag-alaga ng aplikante ay hindi dapat lumagpas sa dalawang daang libong piso (Php 200,000).

Kailangang maipasa ng aplikante ang nakatakdang pagsusulit.
